Upon admission you will be assigned a Case Manager who will follow you throughout your hospital stay. The Case Management department is responsible for helping locate appropriate resources for patients and families in an effort to facilitate discharge planning and ensure patients receive appropriate follow-up care. This can include ordering medical equipment, arranging home health services and setting up doctor’s appointments. A multidisciplinary team including the doctor and a member of the Case Management department will meet weekly to discuss your progress and goals for continued care and ultimate discharge.

The Case Manager serves as the single consistent resource to the family and physician, enhances communication among team members and monitors barriers to the patient’s efficient navigation through the episode of care.

Vibra Hospital/Vista View Care Center’s Finance Department is available to patients and families who may require assistance with medical bills. Please call 303.286.5009 with questions regarding payment options.
